Order A Membership Card
To request a new member card, choose the member who needs a new card from the list below. Your new card will be issued within 7 to 10 working days. If it doesn't arrive, please call us on 134 148.
To request a new member card for anyone under 16 years old, please call us on 134 148.
You can still make an online claim while you're waiting to receive your new card.

Upon activating your membership a card was automatically ordered, and will be sent to the address you provided. You would also have received a welcome email with a link to a digital membership card.
If you have not received your card within 7-10 business days you can re-order your card online. Alternatively call us on 134 148 or email us at oshc@ahm.com.au for assistance.

Thanks for requesting your new member card. If it doesn't arrive within 10 working days, please call us on 134 148.
Once you receive your new member card, you should take care to destroy your old card to ensure you are always using the right one.
Of course, until your new card arrives you can still claim ahm OSHC benefits online. It's quick, convenient and secure!
Card Request Rejected
Thanks for ordering your new membership card. However, we cannot process your request as a new card has already been requested for this member within the last few days.
If this appears to be incorrect or you have any other queries please email us or call our friendly staff on 134 148 during office hours.
If you still have your old membership card, it will continue to work for a limited period; however we recommend that you destroy it as soon as you receive your new one.
And remember, until your new card arrives, you can still claim online. It's quick, convenient and secure!
